Complete your *INTAKE FORM & *WAIVER FORM after booking your appointment.

Upon booking your reserved appointment, you will be required to pay in advance. Refunds are available only if cancellations are made 24 to 48 hours prior, or you may opt to reschedule within that period. Please be aware that another client may be awaiting your cancelled time slot. Advance notice is often necessary for clients to arrange time off work and travel to the location. We appreciate your understanding. Note: No-shows or same-day cancellations will not be refunded.

KEY! Arriving on time ensures your full service, as late arrivals will be accommodated within their original booking slot to respect subsequent appointments. Please note, same-day cancellations will result in forfeiture of rescheduling and refund privileges.

SICK: sneezing, coughing, fever, or body aches in the last two weeks, please ensure you are completely recovered and symptom-free, particularly from a sore throat, before your appointment. Notify us at least 24 hours in advance if you need to reschedule.

IMPORTANT! If you have infection, open sores, cuts, surgery drains, recent surgery in last 6 - 12 months, on medication, or have underline medical conditions that is being monitored by physician, please inform me or the therapist providing your care.

Conditions such as cancer, must be 5 years in remission. Consent Scrip is REQUIRED from your treating physician stating "You are clear to receive Massages / Manual Lymphatic Drainage Services"

If using a credit card there's a minimum of 3.95% fee that will apply (visa, master, etc).
